HEALTH INFORMATION
CONSENT

This page explains the consent connected specifically to health information you choose to discuss during your free consultation. It is not a PAR-Q or the full client-onboarding agreement.

What you are consenting to

During your free consultation, you may choose to tell ArtFitness about health information that is relevant to training or coaching. By actively ticking the consent box on the enquiry form, you agree that ArtFitness may discuss and record the health information you choose to provide for the purpose of assessing your suitability for an ArtFitness training or coaching service.

Why health information may be discussed

A brief conversation about relevant health considerations can help ArtFitness understand whether an in-person training or online coaching service may be suitable, whether further screening will be needed and whether anything should be clarified before you proceed.

The free consultation is not a medical assessment and does not replace the full pre-exercise screening and consent documentation required before training or coaching begins.

You choose what to share

You are not being asked to provide a full medical history in the initial enquiry form. If health information becomes relevant during the consultation, you can decide what to discuss. Please do not send unnecessary medical documents through the enquiry form.

How the information is handled

Health information is sensitive and confidential personal information. Relevant details may be recorded in consultation notes, email or secure business storage so ArtFitness can assess suitability, plan appropriate next steps and keep an accurate record of the conversation.

Access is limited to Arturas and, only where necessary, trusted service providers used for secure communications, storage or website operation. The information will not be sold or used for unrelated marketing.

Withdrawing consent

Where processing relies on your consent, you may withdraw it by contacting hello@artfitness.co.uk. Withdrawal does not affect processing that was lawful before consent was withdrawn.

If information is necessary to assess whether training can be provided safely, withdrawing consent or choosing not to provide the relevant information may mean ArtFitness cannot offer or continue with some services.

Full onboarding comes later

If you and Arturas decide to proceed after the consultation, you will need to complete the required ArtFitness onboarding process before a paid service begins. This includes appropriate pre-exercise screening and PAR-Q, declarations and consent information, reviewing and accepting the Terms & Conditions, and reviewing the Privacy Policy. ArtFitness will review the completed information before confirming that training or coaching can begin.
